In connection with my application for employment, a proposed contract for services, or for housing, I understand that investigative background inquiries may be required, as permitted by law, and which may include without limitation criminal history, civil records, driver’s license verification, driving records, vehicle history, credit history, employment history, educational background, credential verification, employment references, local language media and social media information, directorship information, property ownership information, and such other records that may be requested pursuant to a Background Search Order (“Background Check”) by the Company ordering this Background Check (“Company”) from Diligent Data Services, LLC (“DDS”).
A Background Search Report generated by DDS (“Report”) may also include information as to my character, work habits, work performance and experience along with reasons for termination of past employment from previous employers. DDS and its employees, principals, and suppliers shall not be held liable for the Report’s contents or information if the Report proves to be inaccurate or erroneous based upon the public record and/or data obtained by DDS pursuant to this Background Check and I agree to hold DDS and its employees, principals, and suppliers harmless from any erroneous information provided to the Company by DDS in the Report.
Further, I authorize DDS to request information from various governmental and law enforcement agencies, courts, various other organizations and corporations, former employers, educational institutions, consumer credit information agencies, as well as other organizations or persons which maintain records pertinent to this Background Check and I authorize the release of my information for the purpose of obtaining these records. Upon written request, additional information as to the nature and scope of the Background checks will be provided.
I authorize, without reservation, DDS to furnish any of the above-mentioned pieces of information to the Company ordering this Background Check. Accordingly, I hereby acknowledge and understand that by signing this Release, I waive further notification that the within described information is being reported by DDS to the Company placing the Background Search Order.
